Conditions for obtaining service

  • The applicant must be under compulsory subscription.
  • The brother or sister must be unmarried.
  • The brother or sister must be 70% physically disabled.
  • The brother or sister should not be receiving benefits from other health insurance programs.

Procedures

  1. The applicant shall submit the old cards and the enclosed documents to the Reception Desk officer.
  2. The form and the enclosed documents will be verified for completeness.
    1. If the submitted documents are not complete, the applicant will be asked to update the required documents.
    2. If the submitted documents are complete, the applicant will receive a payment voucher showing the due amount.
  3. The auditor endorses the voucher.
  4. The applicant pays the prescribed fees at the cashier’s counter.
  5. Submit the form and the enclosed documents to the Reception Desk officer.
  6. The Applications Intake officer enters data.
  7. The auditor receives the application and validates information against it.
    1. In case of errors, the application is sent back to the Applications Intake officer to double check the entered data
    2. In case the application is complete, the Audit and Posting officer will post the application.
  8. The application is sent electronically to printing department.
  9. The new card is printed and handed over to the Reception Desk officer.
  10. The Reception Desk officer then gives the new health card to the applicant.

Required Documents

  • A medical report from the district committee stating that the beneficiary is 70% physically disabled.
  • A liabilities certificate to deduct five (10) JODs per month from the employer of the subscriber for every added brother or sister.
  • A copy of the Family Book.
  • One (1) personal photograph.
